Job summary

UNC Health Nash is seeking skilled social work professionals to provide psychosocial assessments and care planning for patients across Nash, Edgecombe, Halifax and Wilson counties. You will document plans and progress, coordinate discharge needs with ED staff, and assist with crisis intervention and community referrals.

Qualifications

• Bachelor’s Degree in Social Work required.
• Master’s Degree in Social Work preferred.
• Minimum of two years paid experience in social work.
• Valid NC Driver’s License, BLS and CPI required.

Responsibilities

• Provide psychosocial assessment, care planning, intervention and evaluation.
• Document plan of care and progress notes on patient responses to interventions.
• Prioritize high-need patients and coordinate discharge planning with ED staff.
• Address crisis situations and coordinate with protective services as needed.
• Collaborate with care teams to support transitions and community referrals.
• Maintain clear communication with patients, families and health professionals.
