Job summary

Ascension in Wisconsin seeks a Case Manager in Brookfield to assess patients, develop care plans, and coordinate interdisciplinary teams from admission to discharge. You will advocate for patients, address emotional and social needs, and ensure respectful navigation of the medical system while collaborating with care transition leaders.

Requires licensure or social work credentialing in Wisconsin and preferred CCMC certification; a high school diploma with experience or an associate degree

Qualifications

• Licensure / Certification / Registration: One or more of the following: Licensed Clinical Counselor, Social Worker, or related credential.
• Education: High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience.

Responsibilities

• Assesses patients' needs and coordinates the overall interdisciplinary plan of care from admission to discharge.
• Develops care plans for patients based on assessment findings and preferences.
• Advocates for patients' needs and ensures the medical system respects their rights and caters to their preferences.
• Maintains ongoing dialog with leadership and care transition team members to ensure effective implementation and reevaluation of care plan.
