Job summary

Ascension is seeking a care coordination professional to oversee an interdisciplinary plan of care from admission through discharge. You will assess psychosocial needs, advocate for resources, and collaborate with care transition teams to optimize health plan implementation and post-discharge services.

The role emphasizes documentation accuracy, appropriate care level recommendations, and alignment with patient/family preferences.

Qualifications

• Education: High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
• Licensure/Certification: Wisconsin Board credentials (Marriage & Family Therapy, Professional Counseling & Social Work) obtained prior to hire date or transfer date.
• Preferred Credential(s): CCMC Case Manager credential.

Responsibilities

• Coordinate the overall interdisciplinary plan of care from admission to discharge.
• Assess, evaluate, and treat legal, neglect, substance abuse, crisis, psychosocial needs and adjustment needs.
• Advocate for resources and removal of barriers to care.
• Maintain dialog with supervisor and care transition team to ensure implementation and reevaluation of the health plan.
• Ensure comprehensive medical record documentation and appropriate level of care recommendations for post-discharge services.
