Job summary

Houston Methodist is seeking a PRN Case Manager RN to plan and coordinate case management for a designated patient population on a unit. You will work with physicians and the interprofessional team to ensure quality, efficient care and appropriate utilization for discharge planning and reimbursement.

Required are comprehensive nursing skills, strong communication, and proficiency with EPIC/MIDAS; experience in discharge planning and managed care is essential.

Qualifications

• Graduate of an education program approved by the credentialing body for required credential(s).
• RN licensure in Texas with PSv Compact; obtain permanent Texas license within 60 days.

Responsibilities

• Collaborates with physicians and health care team to coordinate patient care on assigned units.
• Monitor patient progress, ensure plan of care is patient-focused, and document in EPIC/MIDAS.
• Conduct discharge planning and coordination with social work for reimbursement and transitions.
• Provide education and mentorship to staff; support case management resources.
