Job summary

Houston Methodist is seeking a licensed registered nurse for a PRN Case Manager role to plan case management for a defined patient population on designated unit(s). The CM collaborates with physicians and the care team to ensure medically necessary admissions and efficient, compassionate care while supporting discharge planning and continuity of care.

The CM PRN communicates information to payors for reimbursement and contributes to performance improvement by identifying workflow improvements

Qualifications

• Graduate of education program approved by the credentialing body for the required credential.
• Three years hospital nursing experience including two years in case management.
• RN - Texas state licensure with permanent license within 60 days if establishing residency.

Responsibilities

• Collaborates with physicians and interprofessional team to plan care and discharge.
• Ensures admission and continued stay are medically necessary and communicates with payors.
• Performs review of medical necessity and coordinates discharge planning with external case managers.
• Documents in EPIC and MIDAS and supports performance improvement initiatives.
