Job summary

• Three years hospital clinical nursing experience including two years in case management.

Responsibilities

• Collaborate with physicians and interprofessional team to plan for designated case load.
• Ensure medical necessity for admission, continued stay, and discharge planning.
• Communicate with payors and coordinate with social work for discharge planning and continuity of care.
