Job summary

Houston Methodist is seeking a Case Manager (RN) to assess clinical conditions, plan care transitions, and coordinate discharge planning on designated units. The role partners with physicians and the care team to ensure safe, efficient patient progression and appropriate utilization of resources.

This Exempt position involves weekend coverage (4/10s on Saturdays and Sundays) and collaboration with payors and post-acute providers to secure safe transitions and ongoing quality care.

Qualifications

• RN licensure in Texas or eligibility (PSV compact, must obtain permanent Texas license within 60 days)
• Bachelor’s degree preferred
• Three years hospital nursing clinical experience
• Acute care case management experience preferred

Responsibilities

• Assess patients' clinical condition and plan for case management, care transitions and discharge
• Coordinate with physicians and multidisciplinary team to ensure quality, safe patient care
• Communicate clinical information to payors and post-acute providers for safe transitions
• Participate in discharge planning, throughput and ensuring medical necessity for admission and continued stay
• Engage in population-specific care planning on designated units/services
• Support customer service focus in communication with patients, families and colleagues
• Contribute to reducing length of stay and optimizing resource use
• Assist with performance improvement and process improvement initiatives
