The position emphasizes collaboration with physicians, patients, families, and staff to support quality care, efficient throughput, and safe discharge planning within a 238-bed acute care facility.

Qualifications

• Florida RN license or multistate license required (compact license holders have 60 days to obtain Florida license).
• BSN degree preferred.
• BLS-Healthcare Provider certification required within 30 days of start date.

Responsibilities

• Performs a comprehensive assessment of psychosocial and medical needs of patients.
• Advocates for patients while taking a leadership role with the interdisciplinary team.
• Coordinates plan of care.
• Acts as a liaison through effective and professional communications with physicians, patients, families, hospital staff, and outside agencies.
• Performs utilization management reviews and communicates information to third party payors.
• Facilitates patient throughput with an ongoing focus on quality and efficiency.

Education

Florida RN license or multistate license

BLS Provider

Case Management certification preferred
