Qualifications

• Licensed as a Registered Nurse in Florida.
• 1 year of RN experience in an acute care setting is required.
• BSN is preferred.
• Certification in Case Management, Nursing or Utilization Review is preferred.
• 1 year Case Mgmt experience Required.

Responsibilities

• Performs comprehensive assessment of psychosocial and medical needs of patients.
• Advocates for patients while leading the interdisciplinary team.
• Coordinates plan of care.
• Acts as liaison with physicians, patients, families, hospital staff and outside agencies.
• Performs utilization management reviews and communicates with third party payors.
• Facilitates patient throughput with focus on quality and efficiency.
