UCF Lake Nona Hospital in Orlando, FL is seeking a dynamic Case Management RN to coordinate the interdisciplinary plan of care, assess medical necessity, admission status, and resource utilization, while promoting quality outcomes and efficient patient throughput.

The role requires Florida RN licensure, 1 year acute care experience, BSN preferred, and certifications in Case Management or Utilization Review preferred.

Qualifications

• Licensed as a Registered Nurse in the State of Florida or able to obtain licensure prior to start
• 1 year of RN experience in an acute care setting is required
• BSN is preferred
• Certification in Case Management, Nursing or Utilization Review is preferred
• 1 year Case Mgmt experience Required

Responsibilities

• Performs a comprehensive assessment of psychosocial and medical needs of patients
• Advocates for patients while assuming a leadership role with the interdisciplinary team
• Coordinates plan of care
• Acts as a liaison through effective and professional communications with physicians, patients, families, hospital staff, and outside agencies
• Performs utilization management reviews and communicates information to third party payors
• Facilitates patient throughput with an ongoing focus on quality and efficiency
