Job summary

HCA Florida North Florida Hospital in Gainesville, FL is seeking an RN Case Manager to coordinate patient care across the continuum and partner with physicians and the care team to ensure safe transitions and optimal outcomes.

You will develop individualized care plans, assess psychosocial needs, educate patients and families, and support discharge planning while upholding the hospital's values of compassion and excellence.

Qualifications

• Bachelor's degree or RN diploma or associate degree is required.
• RN license required.
• No travel required.
• 2 years of RN experience required.
• 2 weekend days of call per month.

Responsibilities

• Guide patients and families through orientation, discharge criteria, and rights.
• Coordinate education for patients, families, and caregivers.
• Develop individualized treatment plans reflecting patient needs and recovery goals.
• Complete psychosocial assessments and discharge planning evaluations.
• Communicate needs across the care team and collaborate with Rehab Program Director and interdisciplinary team.
• Promote hospital values of compassion, respect, and excellence.
