Qualifications

• Certification requirements: BLS and ACM or CCM to start.
• Licensure: Master’s in Social Work or RN licensed in Virginia.
• Experience: 5 years of acute care case management in an acute healthcare setting.
• Education: BSN or MSW.

Responsibilities

• Oversees daily operations of Case Management on the unit and discharge planners’ patient care plans.
• Collaborates with physicians, nurses and the multidisciplinary team to ensure appropriate patient care management.
• Manages discharge planning and continuity of care for assigned patients in acute and post-acute settings.
• Guides and trains discharge planners; participates in MDRs and manages assigned patients.
