Key duties include discharge planning, care coordination across acute and post-acute settings, and serving as a liaison between patients, families, and healthcare professionals.

Qualifications

• MSW degree required.
• 1 year experience in clinical care or clinical case management.
• Basic Life Support certification upon start.

Responsibilities

• Assess patients' biopsychosocial needs and coordinate care with the care team.
• Communicate with patients, families, and team members about care plans.
• Coordinate discharge planning and continuation of care across settings.
• Advocate for patient empowerment and timely access to services.
• Document discharge planning details in the medical record per standards.
