Job summary

Inova Fair Oaks Hospital is seeking an Experienced Manager Case Management to lead the Case Management Team on a full‑time, day shift with weekend rotations. The role offers hybrid capabilities and opportunities to participate in clinical performance improvement.

You will supervise staff, coordinate patient care progression across the continuum, and partner with physicians and care teams to ensure safe discharge planning and transitions of care.

Qualifications

• Bachelor's Degree in Nursing (BSN) or MSW required.
• Three years of case management experience plus two years of supervisory/manager experience.
• Registered Nurse license in Virginia required at start.
• Basic Life Support (BLS) required at start.
• ACM/CCM or LCSW or equivalent qualifications/Education within 12 months.

Responsibilities

• Participates in interviewing/hiring process and provides feedback on candidates.
• Ensures referrals to next level of care are initiated in a timely manner.
• Leads family meetings and coordinates care with interdisciplinary teams.
• Develops and communicates individualized continuing care plans.
• Supervises staff and evaluates performance to close knowledge gaps.
