• Conduct discharge follow-up calls as needed with center staff.
• Assist families in resolving issues and escalate as appropriate.
• Manage DME inventory and track issues.
• Help track center metrics and ensure policy compliance.
• Share clinical information with third-party payers when needed.
• Engage in center activities to improve child and family health.
