Job summary

BayCare Health System is seeking a dedicated Case Management Assistant to support patient access to services and resources across the care continuum. The role focuses on facilitating timely discharges, coordinating with hospital staff and community partners, and educating patients about discharge appeals.

Qualifications

• 1 year in a medical-related field required or
• 1 year of customer service experience required

Responsibilities

• Support seamless patient transitions from inpatient to appropriate services
• Maintain frequent contact with patients, families, hospital staff and community resources

The Case Management Assistant Responsibilities Include

• Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute care
• Responsible for assisting with distributing regulatory documents to identified patients
• Educating patients about the appeal process related to their discharge
• Performing multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service
• Requires frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives

• Schedule: Monday - Friday 8:00 AM - 4:30 PM
• Weekend Requirement: Occasional
• On Call: No

Education

• Required High School or Equivalent
• Preferred Associate's

Experience

• Required 1 year in Medical Related Field OR
• Required 1 year Customer Service skills
