Job summary

BayCare in Dunedin, FL is seeking a Case Management Assistant (PRN) to help coordinate patient access to services and resources. The role involves distributing regulatory documents, educating patients on discharge appeals, and ensuring smooth transitions from hospital to community supports.

Ideal candidates will have at least 1 year of experience in a medical-related field and a high school diploma or equivalent, with a preference for an associate's degree.

Qualifications

• Required high school diploma or equivalent.
• Preferred associate's degree.
• Minimum 1 year of experience in a medical-related field.

Responsibilities

• Assists with completing tasks to facilitate patient and family access to agencies, transportation, and services within and beyond acute care.
• Responsible for assisting with distributing regulatory documents to identified patients.
• Educating patients about the appeal process related to their discharge.
• Performing multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service.
• Requires frequent contact with patients and patient representatives including hospital staff, community resource agencies and payor representatives.

Case Management Assistant responsibilities

• Status: PRN As needed
• Schedule: 8:00 AM – 4:30 PM
