Job summary

BayCare is seeking a Case Management Assistant to help patients and families access necessary services and resources throughout the discharge process. You will assist with distributing regulatory documents and educating patients about discharge appeals, ensuring smooth transitions from hospital to community care.

The role emphasizes collaboration with hospital staff and Payor representatives, requiring strong communication and organizational skills and experience in medical settings.

Responsibilities

• Assist with tasks to facilitate patient and family access to agencies, transportation, and services within and beyond acute care.
• Distribute regulatory documents to identified patients.

• Support seamless transition of patients from inpatient to community services.
• Maintain frequent contact with patients, patient representatives, hospital staff, community resource agencies and payor representatives.

The Case Management Assistant responsibilities include:

• Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute care
• Responsible for assisting with distributing regulatory documents to identified patients
• Educating patients about the appeal process related to their discharge
• Performing multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service
• Requires frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives

Position details:

• Location: St. Joseph's Main and Children, FL
• Status: PRN As needed
• Schedule: 8:00 AM - 4:30 PM
• Weekend Requirement: Occasional
• On Call: No
