Job summary

BayCare Health System is seeking a Hospital Care Coordinator-RN Case Management to join a large, integrated health system providing discharge planning and patient advocacy across the continuum of care.

The position requires an RN license and 2 years of acute care experience, with ACM/CCM certifications preferred. Work is conducted at St Josephs South Hospital in Riverview, FL, with a standard 40-hour work week and occasional weekend coverage.

Qualifications

• Current RN license is required.
• ACCM (Case Management) and CCM (Case Manager) certifications preferred.
• Minimum 2 years RN experience in Acute Care or equivalent.

Responsibilities

• Provides linkage, monitoring, planning and advocacy for our population to achieve and maintain maximum functioning.
• Effectively coordinates discharge planning through assessment, referral, and interdisciplinary planning.
• Manages assigned caseload with flexibility to adapt to changing priorities in a fast-paced environment.
• Promotes quality, cost-effective outcomes by utilizing available resources and collaborating with the care team.

The Hospital Care Coordinator-RN Case Management Responsibilities Include

• Provides linkage, monitoring, planning and advocacy for our population to achieve and maintain maximum functioning.
• Possesses a clear understanding of the available resources to promote quality cost effective outcomes.
• Effectively maintains and manages assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served.
• Coordinates discharge planning efforts through assessment, referral, and interdisciplinary planning.
• Ability to adjust to changing priorities in a fast-paced environment.

Position Details

• Location: St Josephs South Hospital - Riverview, FL
• Status: full time, 40 hours per week. Exempt.
• Schedule: Monday - Friday 8:00 AM - 4:30 PM
• Weekend Requirement: Occasional
• On Call: No

Certifications And Licensures
