Job summary

BayCare Health System is seeking a Case Management Assistant in Florida. The role supports patient and family access to services, distributes regulatory documents, and educates patients on discharge appeal processes. The position involves coordinating transitions from inpatient care and requires frequent contact with patients, staff, and community resources.

PRN as needed with daytime hours. Required a high school diploma or equivalent; one year in a medical-related field or customer service is

Qualifications

• High school diploma or equivalent required; associate's degree preferred.
• At least 1 year in a medical-related field or 1 year of customer service experience.
• PRN position; schedule 8:00 AM–4:30 PM; Florida location.

Responsibilities

• Assist with tasks to facilitate patient and family access to agencies, transportation, and services within and beyond acute care.
• Distribute regulatory documents to identified patients.
• Educate patients about the appeal process related to their discharge.
• Support seamless transition of patients from inpatient to discharge, using current hospital processes.
• Maintain frequent contact with patients, family, hospital staff, community resource agencies, and payor representatives.

The Case Management Assistant Responsibilities Include

• Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute care
• Responsible for assisting with distributing regulatory documents to identified patients
• Educating patients about the appeal process related to their discharge
• Performing multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service
• Requires frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives

• Location: St. Joseph's Main and Children, FL
• Status: PRN As needed
• Schedule: 8:00 AM – 4:30 PM
• Weekend Requirement: Occasional
• On Call: No
