Job summary

Radiant Health in Indiana is seeking a Case Coordinator to provide comprehensive case management and support for adults with mental health and/or substance use disorders in the community. The role emphasizes assessments, service planning, care coordination, and advocacy to enhance recovery and independence.

You will work closely with clients, families and community partners in a mission-driven environment, with required immunization and health protocols to ensure a safe workplace.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or a related field.
• Minimum of 2 years of experience in case management, or a related field.
• Relevant licensure or certification preferred.
• CPR and First Aid certification preferred.
• Valid driver’s license and reliable transportation for community-based work.

Responsibilities

• Provide comprehensive case management and support services to individuals with mental health and/or substance use disorders in the community.
• Conduct assessments and develop individualized service plans.
• Coordinate care and advocate for clients’ access to community resources.
• Collaborate with clients, families and community partners to promote recovery and well-being.

This role involves conducting assessments, developing individualized service plans, coordinating care, and advocating for clients' access to community resources.
