Job summary

Radiant Health is seeking a Case Coordinator to provide comprehensive case management and support to individuals with mental health and/or substance use disorders in the Marion community. The role involves assessments, service planning, care coordination, and advocacy for access to resources.

You will collaborate with clients, families, and partners to promote recovery, independence, and well‑being, contributing to a mission‑driven team that serves Grant and Blackford Counties in Indiana.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or related field.
• Minimum of 2 years of experience in case management.
• Valid driver’s license and reliable transportation.
• CPR and First Aid certification preferred.

Adult Services:

Case Coordinators are responsible for providing comprehensive case management and support services to individuals with mental health and/or substance use disorders living in the community. This role involves conducting assessments, developing individualized service plans, coordinating care, and advocating for clients' access to community resources. The Case Coordinator will work collaboratively with clients, families, and community partners to promote recovery, independence, and overall well-being.
