Job summary

Radiant Health is seeking a Case Coordinator in Marion, IN to empower youth, families, and adults with individualized training across homes, schools, and communities. You will help clients build daily living and social skills, promoting independence in a supportive setting.

You’ll collaborate with families and service providers, deliver group and one-on-one sessions, and maintain thorough documentation while staying current with best practices through ongoing training.

Qualifications

• Bachelor’s degree preferred in psychology, social work, education, rehabilitation services, or related field.
• CRC or Life Skills Coach certifications are a plus.
• Valid driver’s license, insurance and reliable transportation to visit clients in the community.

Responsibilities

• Develop and implement personalized training plans based on clients' goals and needs.
• Provide one-on-one and group sessions focusing on daily living and social skills.
• Apply evidence-based practices to support learning and growth.
• Offer mentorship and encouragement in a supportive environment.
• Help clients apply new skills in real-world situations to build confidence.
• Collaborate with families, caregivers, and service providers for holistic care.
• Maintain accurate documentation, progress notes and reports.
• Connect clients with community resources and promote participation in activities.
