SJRC Texas is seeking a Case Care Specialist for the Belong Network in the San Antonio area. The role focuses on coordinating services to ensure safety, permanency, and well-being of placements, working with providers to ensure required services are delivered.

You will collaborate with DFPS, network providers, and families, documenting interactions and maintaining records. This position operates in a hybrid setting and requires local residence within the service area.

Competenze

Associate’s degree with two years of related work experience or 60 credit hours with two years of experience or 90 credit hours with one year of experience.
Preferred: Bachelor’s degree or Master’s degree in Child Welfare or related field.

Mansioni

Coordinate admissions, case planning, extensions, and discharges of child/youth with provider network.
Collaborate with SJRC Texas/Belong network providers and community partners to deliver services.
Document contacts and case activity logs; maintain client files.
Maintain relationships with DFPS, the provider network, and children and youth.
Provide support to intake and placement, including transportation.
Engage and communicate effectively with diverse cultures and communities.
