Job summary

Advocate Health is seeking a dedicated Home Health Aide to provide in-home personal care under an RN plan of care. The role involves assisting with daily living activities, vital signs, and documentation, while supporting patients in a respectful, empathetic manner. A valid driver’s license is required.

You will work closely with a care team to ensure comfort and safety for patients in their homes, with attention to age-appropriate needs and timely communication of changes in condition.

Responsibilities

• Make home visits and provide personal care per care plan.
• Perform routine personal care tasks (bathing, dressing, grooming, etc.).
• Record patient vital signs and document care provided.
• Assist with transfers, mobility, and positioning.
• Assist with meals, monitoring urine tests, and planning activities.
• Conduct non-routine care with RN instruction as needed.
• Perform light home management tasks as required.
• Communicate with physicians and care team; document updates.
• Maintain professional and compassionate interactions with patients and families.
• Participate in chart audits and quality improvement activities.
