Job summary

State Personnel System in Florida is seeking a CARES ASSESSOR to perform comprehensive assessments for nursing facility placement and home and community-based services. The role evaluates Level of Care and makes placement recommendations within an interdisciplinary team.

Must maintain accurate medical information, ensure PASRR compliance, and participate in professional development while supporting disaster recovery efforts across Florida. Attendance is essential.

Qualifications

• Knowledge of Level of Care rules and CARES policies and procedures.
• Understanding aging and effects of chronic illness.
• Ability to compile, organize, and analyze client information.

Responsibilities

• Coordinates and conducts comprehensive assessments for nursing facility placement and home and community-based services.
• Evaluates Level of Care and recommends the most appropriate placement as part of an interdisciplinary team.
• Maintains accurate medical information and timely data collection in CIRTS per CARES policies.
• Collaborates with Title XIX Level of Care decisions with physicians and other professionals.
• Ensures PASRR processes are completed before Level of Care is issued.
• Monitors placement decisions and communicates with the long-term care network and providers.
