Job summary

The State Personnel System is seeking a CARES ASSESSOR to conduct comprehensive assessments for nursing facility placement and home and community-based services in Cocoa, FL. Salary ranges from $42,487.32 to $46,736.04 annually and requires reporting to the DOEA office serving the area.

Responsibilities include interdisciplinary team participation, PASRR compliance, data management in CIRTS, and ensuring timely eligibility determinations. Travel within the planning area required.

Qualifications

• Four (4) or more years of professional experience in the field of community and social services.
• A bachelor’s degree or higher from an accredited college or university with a major in the field of community and social services.
• A combination of education and professional experience totaling four (4) or more years.

Responsibilities

• Coordinates and conducts comprehensive assessments for nursing facility placement and home and community-based services.
• Evaluates Level of Care and recommends the most appropriate placement.
• Maintains accurate medical information and regular documentation of activities in CIRTS.
