Job summary

Community and Social Services seeks a CARES Assessor to conduct comprehensive assessments of applicants for nursing facility placement and home and community-based services for the Department of Elder Affairs. The role involves evaluating Level of Care, coordinating with interdisciplinary teams, and maintaining accurate client data in CIRTS.

This on-site position requires travel within the assigned planning and service area and adherence to state and federal regulations governing long-term care

Qualifications

• Four or more years of professional experience in community and social services or a bachelor's degree in the field.
• Knowledge of Level of Care rules, CARES policies and procedures, and aging process.
• Strong written and oral communication with ability to compile, organize, and analyze client information.
• Proficiency with Microsoft Windows and Office Suite products (Word, Excel, Outlook, OneDrive, SharePoint).
• Valid Florida driver's license and ability to operate a private motor vehicle for routine travel.
• Ability to work in a large open office environment and maintain confidentiality.

Key qualifications:

• Four or more years of professional experience in community and social services, or a bachelor's degree in the field, or equivalent combination of education and experience
• Knowledge of Level of Care rules, regulations, CARES policies and procedures, and aging process
• Strong written and oral communication skills with ability to compile, organize, and analyze client information
