Job summary

The Social Worker (Program Coordinator) within the Caregiver Support Program at the Sioux Falls VA Health Care System leads a team and coordinates care for Veterans’ family caregivers. Education programs for Veterans and caregivers are developed and maintained, with an emphasis on guidelines, mental health, and psychosocial support.

Qualifications

Education or degrees recognized by the U.S. Department of Education from accredited institutions will qualify for Federal employment.
Foreign education may be considered with a Certificate of Foreign Equivalency accompanying the transcript.

Responsibilities

Oversees service delivery and coordination of care activities for family caregivers who provide personal care services to eligible Veterans.
Coordinates program implementation with multiple clinical services across campus locations including nursing and mental health.
Manages staff, assigns work, conducts performance evaluations, and handles training and awards.
Establishes and maintains an ongoing education program for Veterans, caregivers, and staff on guidelines and psychosocial issues.
Maintains a network of referral resources and serves as the Sioux Falls VA HCS POC for PCAFC/PGCSS inquiries.
