Job summary

The Sioux Falls VA Health Care System is seeking a Community Care Home and Community Based Services RN Case Manager to help Veterans remain independent at home. You will coordinate skilled home care, adult day health care, and respite services while guiding Veterans through care transitions.

Work with VA teams and community providers to connect Veterans with personalized services that honor their preferences and strengthen quality of life.

Qualifications

• English language proficiency required for direct patient care.
• Proficient in written and spoken English as per VA requirements.
• Graduate of an approved nursing program accredited by ACEN or CCNE.
• State nursing license in good standing; bridge options may apply.
• Current, full, active, and unrestricted RN license in the U.S.
• Foreign nursing education may require degree equivalency validation.
• GNT exception allows temporary appointment while pursuing licensure.

Responsibilities

• Make a meaningful difference by helping Veterans remain at home and in their communities.
• Coordinate comprehensive home and community-based services to support independence.
• Manage complex cases with personalized care plans and ongoing monitoring.
• Oversee coordination of skilled and non-skilled home care, adult day health, and respite services.
• Collaborate with interdisciplinary VA teams and community providers for integrated care.
• Facilitate transitions between VA and community services and engage families in care planning.
• Guide providers through VA processes to ensure timely service delivery.
