Job summary

The Sioux Falls VA Health Care System seeks a Community Care Home and Community Based Services RN Case Manager to help Veterans remain independent at home. You will coordinate home care, adult day health care, and respite services, guiding Veterans through care transitions with VA and community providers.

You will work to honor Veterans’ preferences, collaborate with interdisciplinary teams, and promote quality of life in the home and community setting.

Qualifications

• English language proficiency required.
• Current RN license in good standing with state boards.
• Preferred experience in home health or case management.

Responsibilities

• Coordinate comprehensive home and community-based services to help Veterans stay independent at home.
• Manage care transitions and oversee skilled and non-skilled home care, adult day health care, and respite services.
• Collaborate with interdisciplinary VA teams and community providers to ensure timely, Veteran-centered care.
• Guide care planning with Veterans, families, and caregivers reflecting personal goals and preferences.
• Navigate VA care coordination processes and service authorization to support timely delivery of services.
• Evaluate services for effectiveness and optimize resource use across care settings.
