St. John's United seeks a Family Caregiver Liaison to support family caregivers and older adults through the RAAN grant, a 3-year program running 2025–2027.

You will expand caregiver engagement, strengthen referral pathways, and help build sustainable service processes in community settings. You will be the primary contact for caregivers, coordinating with partners, conducting assessments, and promoting resources.

Qualifications

• Associate degree in healthcare or related field, or equivalent experience.
• Montana CNA certification current or obtainable within 120 days.
• Valid driver's license and clean driving record.

Responsabilités

• Serve as primary contact for caregivers, providing guidance, emotional support, and education.
• Assess caregiver and client needs to identify required support and plan services.
• Coordinate referrals and ensure continuity of care across providers.
• Promote services and engage community partners to expand caregiver networks.
