Continuum - St. Louis Home Care is seeking a Care Navigator to support older adults with dementia and their families from a remote, nationwide location. You will provide resource referrals, counseling, and coordinate transitions of care with healthcare providers and caregivers.

Qualifications

• BSW or MSW degree (or equivalent) required.
• Experience with older adults and memory impairment.
• Pass a background check.
• Positive, engaging, proactive approach toward clients and families.

Responsibilities

• Communicate with clients, families, healthcare providers, and resources.
• Coordinate transitions of care for clients.
• Identify resources to enhance independence and support for clients and caregivers.
• Understand dementia and its challenges for clients and caregivers.
