Job summary

The role focuses on expanding caregiver engagement, strengthening referral pathways, and creating sustainable service processes across Yellowstone County. You will coordinate with providers, education teams, and community partners to ensure access to respite and resources for at-home care.

Qualifications

• Associate’s degree in healthcare or related field.
• Must possess Montana State Nurse Aide Certification (CNA) or obtain within 120 days.
• Strong interpersonal, communication, and customer service skills.
• Experience with CRM systems, data tracking, and basic computer applications.

Responsibilities

• Provide guidance, emotional support, and education to family caregivers.
• Assess caregiver and client needs; develop individualized support plans and connect to resources.
• Coordinate referrals and maintain care continuity across providers and programs.
• Engage with community partners; promote services and support caregiver networks.
