Qualifications

• Associate degree or equivalent experience in healthcare, social services, or related field.
• Certified Nursing Assistant (CNA) required or obtained within 120 days.
• Valid driver’s license with clean driving record.

Responsibilities

• Serve as the primary contact for family caregivers, offering guidance, emotional support, and education.
• Conduct caregiver and client assessments to identify needs, capacity, and social determinants of health.
• Develop individualized support plans and connect caregivers to community resources and services.
• Provide ongoing follow-up, check-ins, and advocacy within healthcare and social services systems.
• Maintain accurate caregiver and client records in CRM systems and track outcomes.
• Assist caregivers in accessing respite services and occasional direct respite care when needed.
