Job summary

Primecare Home Care Services Inc. is seeking a Care Support Specialist to coordinate caregiver recruiting, onboarding, credentialing, scheduling, and ongoing compliance while supporting client onboarding and service changes.

You will work closely with Care Relationship Managers, Nursing, HR/Recruiting, Billing, and other partners to ensure readiness for assignment and timely service initiation. The role requires a High School Diploma or GED, 1+ year in home care or related fields, and strong

Qualifications

• High School Diploma or GED required.
• 1+ year of experience in home care, healthcare, staffing, scheduling, or a related service environment.
• Experience handling multi-line phone systems and managing a high volume of inbound and outbound calls.
• Strong customer service, communication, organizational, and documentation skills.
• Ability to manage multiple priorities and maintain accurate records in a fast-paced environment.
• Valid driver’s license and reliable transportation for required field visits.
• Preferred qualifications include CNA credential; Bachelor’s degree or 2+ years of related experience.

Responsibilities

• Maintain a dedicated caseload of caregivers and serve as their main point of contact for employment, compliance, and performance matters.
• Conduct regular home visits to observe caregiver performance and ensure quality of care.
• Complete performance reviews and coach or implement corrective action when needed.
• Support caregiver retention through regular communication and recognition.
• Coordinate onboarding for direct care team members and support HR/Recruiting as needed.
• Track credentials, renewal reminders, and ensure compliance with state and Medicaid requirements.
• Provide onboarding assistance to ensure caregivers are trained and ready to begin service.
• Conduct field oversight visits and collect feedback to drive continuous improvement.
