Job summary

Independent Home Health LLC in Brookville, PA is seeking a dedicated Home Care Scheduler to coordinate client visits and manage daily and weekly schedules. You will ensure coverage according to each Plan of Care (POC) and maintain seamless communication between management, caregivers, and partners.

In this on-site/hybrid role, you will monitor EVV compliance and keep profiles current in HHAeXchange, while supporting admissions and recruitment to optimize caregiver placement.

Qualifications

• 1–2 years in scheduling or administrative roles within home care or medical office setting.
• Strong computer literacy (Google Workspace) and experience with scheduling software (HHAeXchange preferred).
• Familiarity with EVV systems is a major plus.

Responsibilities

• Schedule daily/weekly client visits ensuring coverage per Plan of Care (POC).
• Maintain accurate records in HHAeXchange and keep profiles current.
• Monitor EVV daily to ensure clock-in/out compliance and adherence to ODP regulations.
• Act as first point of contact for call-outs and emergencies, close care gaps to maintain continuity of care.
• Collaborate with admissions and recruitment to optimize caregiver placement.
