Job summary

Vitality Living in Elizabethtown, IL is seeking a dedicated Caregiver/Personal Care Specialist to support residents in daily activities and wellness programs. You will collaborate with the Wellness Director and care team to implement individualized service plans and respond to resident needs with compassion.

The role emphasizes effective communication in English, adherence to state health requirements, and participation in onboarding and ongoing training.

Qualifications

• Must be at least 18 years old.
• Demonstrate ability to communicate effectively in English, both verbally and in writing.
• Maintain any certification as required by state or provincial regulations
• Meet state or provincial health related requirements

Responsibilities

• Assist residents with activities of daily living and simple treatments as indicated in service plans
• Assist in development and revision of service plans by informing supervisor of resident needs and preferences
• Observe and report resident problems to LPN or Medication Tech/Aid
• Attend onboarding, annual and in-service training and team meetings as requested
• Perform other duties as assigned by Wellness Director
