Vitality Living Elizabethtown is seeking a dedicated Caregiver/Personal Care Specialist to join our Elizabethtown community. You will assist residents with daily activities, support care plans, and participate in ongoing training to ensure high-quality resident care.

Qualifications

• Must be at least 18 years old.
• Must communicate effectively in English, both verbally and in writing.
• Maintain any state-required certifications.
• Meet state health-related requirements.

Responsibilities

• Assist residents with activities, simple treatments, and emotional health issues per service plans.
• Assist in development and revision of service plans by communicating resident needs and successful approaches to supervisors.
• Observe and report resident problems to the LPN or Medication Tech/Aid as applicable.
• Participate in onboarding, annual and in-service training, and team meetings as requested.
• Perform other duties as assigned by Wellness Director.

Caregiver/Personal Care Specialist Responsibilities:

• Assist residents with performance of activities, simple treatments, and emotional health and/or problem behavioral issues as indicated in service plans
• Assist in development and revision of service plans by informing supervisor of current resident needs and preferences, as well as successful service approaches
• Observe and report resident problems to LPN (if on duty) or Medication Tech/Aid
• Attend and participate in onboarding, annual and in-service training and team member meetings, as requested
• Perform other duties as assigned by Wellness Director
