Job summary

Greenbrier Senior Living in Kingston, TN invites you to join Sycamore Trace as a Caregiver. You will follow individualized care plans, assist with daily living activities, and support residents with mobility and personal care.

Qualifications

• High school diploma or GED is required.
• CNA certification preferred (or state-approved Nursing Assistant Training).
• CPR/First Aid certification may be required.
• Strong communication and teamwork skills.
• Ability to lift up to 50 lbs and assist residents safely.

Responsibilities

• Follow individualized care plans (ISP) for each resident.
• Monitor residents and report changes promptly.
• Document care and observations accurately.
• Assist residents with ADLs (bathing, dressing, toileting, oral care).
• Support mobility, transfers, and ambulation with proper techniques.
• Encourage participation in activities.
• Provide behavioral support, cues, and redirection as needed.
• Respond to resident calls promptly and professionally.
• Assist with transportation within the community and to appointments.
• Perform additional duties as assigned.

What You’ll Do

• Follow individualized care plans (ISP) for each resident
• Monitor residents for changes in condition and report concerns promptly
• Document care and observations accurately throughout each shift
• Assist residents with activities of daily living (ADLs) including bathing, dressing, toileting, and oral care
• Support mobility, transfers, and ambulation using proper techniques and equipment
• Encourage and assist residents in participating in activities
• Provide compassionate behavioral support, cues, and redirection as needed
• Respond to resident calls promptly and professionally
• Assist with transportation within the community and to appointments when needed
• Perform additional duties as assigned

What You Bring

• High school diploma or GED required
• CNA certification preferred (or completion of a state‑approved Nursing Assistant Training Program if required)
