Responsibilities

• Build a professional one-on-one relationship with clients to aid in employment.
• Provide consistent case management and follow-up.
• Ensure documentation is completed and received.
• Maintain organized database of clients and documentation of contacts.
• Provide assessments of work and referrals to services when needed.
• Document all participant contacts and activities in case notes.
