Job summary

Reliant Rehabilitation is seeking a Speech-Language Pathologist to provide therapy and related services to patients in skilled nursing facilities. You will assess, treat, and monitor communication, cognitive-linguistic, and swallowing disorders, while maintaining positive collaboration with facilities and clients in line with Reliant guidelines.

You will develop initial and ongoing treatment plans, participate in discharge planning, and ensure compliance with state and federal regulations.

Qualifications

• Master’s Degree in Speech Language Pathology (or equivalent).
• Valid state licensure as a Speech-Language Pathologist (where required).
• Excellent communication and documentation skills.

Responsibilities

• Provide Speech-Language Pathology therapy and related services to patients in SNF settings.
• Develop and implement treatment plans and discharge plans.
• Monitor progress and adjust plans to achieve goals.
• Maintain professional conduct and compliance with state and Reliant policies.
• Read/write/speak English proficiently.
