Job summary

Reliant Rehabilitation is seeking a licensed Speech-Language Pathologist to provide comprehensive SLP therapy in skilled nursing facilities. You will screen, evaluate, and treat patients, develop ongoing plans of care, and coordinate discharge planning with care teams.

You will maintain high professional standards, adhere to regulations, and participate in continuing education that supports growth within Reliant’s clinical ladder.

Qualifications

• Master’s degree in Speech-Language Pathology.
• Valid state license for Speech-Language Pathologist.
• Computer literacy and proficiency in clinical documentation.

Responsibilities

• Provide Speech-Language Pathology therapy and related services to patients.
• Develop initial and ongoing treatment plans of care and discharge planning.
• Manage case load and coordinate with SNF partners to ensure quality patient care.
• Maintain professional conduct and comply with state and federal regulations.
