Enhabit Home Health & Hospice is seeking dedicated healthcare professionals to support patients in the home setting. This role emphasizes patient-centered care, adherence to clinical guidelines, and collaboration with physicians and care teams to optimize outcomes and seamless transitions from hospital to home.

Qualifications

• Must be a graduate of an approved school of nursing, therapy or social work.
• Must be licensed in the state where they currently practice.
• Must have two years' demonstrated field experience.
• Must have demonstrated knowledge of federal, state, and local laws and regulatory guidelines governing Medicare certified home health and hospice operations.
• Must have basic technology skills, including operation of a mobile device.

Responsabilités

• Assists patients in the process of navigating post-acute care to improve outcomes and referral source satisfaction.
• Develops transition plans that are patient-centered and promote quality and efficiency in home-based care.
• Represents area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and executive level opportunities.
