AOASCC is seeking a compassionate Specialized Care Manager to coordinate transitions from institutional care to community living under the Money Follows the Person program. You will conduct assessments, develop care plans, and provide ongoing advocacy and case management to support independence for older adults and individuals with disabilities in Connecticut.

Formación

• Bachelor's degree or higher in a related field.
• Minimum of two years of experience in case management, care management, social services, discharge planning, healthcare, or LTSS.
• Strong assessment, documentation, organizational, and problem-solving skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Valid driver's license and reliable transportation for community visits.

Responsabilidades

• Establish timely contact with assigned consumers and authorized representatives.
• Conduct comprehensive assessments of medical, functional, social, and environmental needs.
• Gather and maintain documentation, including informed consent forms and assessments.
• Identify barriers to community transition and develop strategies to address them.
• Develop individualized, person-centered care plans and coordinate services with providers, agencies, and family.
• Monitor service delivery and adjust plans to meet participant needs.
• Coordinate discharge planning and transition meetings; ensure necessary authorizations and documentation.
• Provide post-transition follow-up and quality monitoring; maintain regular contact with participants.
