Agency on Aging of South Central Connecticut (AOASCC) is seeking a Community First Choice Care Manager to support older adults and individuals with disabilities. This role focuses on developing, implementing, and monitoring person-centered care plans that promote independence and community living.

You will collaborate with clients, families, healthcare providers, and community partners to access essential services, adapt plans as needs change, and ensure compliant documentation and outcomes.

Qualifications

• Bachelor’s degree in Social Work, Nursing, Psychology, or related human services field.
• At least two years of experience in care management or similar role.

Responsabilités

• Conduct comprehensive assessments to inform individualized care plans.
• Develop, coordinate and monitor in-home and community-based services per CFC standards.
• Collaborate with healthcare providers, families, and community partners.
• Maintain accurate documentation of assessments and progress.
