Job summary

Allegheny Health Network is seeking a Medical Social Worker to collaborate with an interdisciplinary team to improve care quality, patient experience, and population health by addressing social determinants of health.

You will assess patients, develop discharge plans, coordinate with post-acute providers, and counsel patients and families on care options. The role emphasizes crisis interventions and ethical, compliant practice within HIPAA guidelines.

Qualifications

• Experience in a hospital or health care setting.
• Master's degree in Social Work (Incumbents in the role on or before 4/21/2025 have 6 months from hire date to obtain).
• PA LSW and/or LCSW required (Incumbents in the role on or before 04/21/2025 have 12 months from hire date to obtain).

Responsibilities

• Contributes to and/or completes initial and ongoing comprehensive assessment. Provides interventions and implements recommendations after engaging patients and their caregivers/families.
• Collaborates to provide the safest transition plan for assigned patients to ensure a timely discharge and connect with post-discharge care providers.
• Educates patient, family/caregiver and physician regarding most appropriate level of care post discharge and how to access community support.
• Provides crisis interventions in cases involving various forms of abuse, mental health, substance use disorders, and surrogate decision making.
