Job summary

Qualifications

• Bachelor’s degree in healthcare field required.
• 2 years of healthcare experience or <1 year as a new graduate.
• Strong communication and collaboration skills with diverse teams.
• Knowledge of HIPAA, insurance rules, and care coordination.

Responsibilities

• Coordinate post-acute services and arrange follow-up appointments.
• Obtain payer authorization for post-discharge services; submit clinical info timely.
• Document discharge planning activities in the medical record.
• Act as liaison among patients, families, providers, payers, and community agencies.
• Support transportation and intra-facility transfer coordination.
