Job summary

Kennedy University Hospitals, Inc. seeks a Discharge Planner in Cherry Hill, NJ to coordinate safe and efficient transitions of care from hospital to home or another setting.

You will work with care managers, providers, and interdisciplinary teams to schedule follow-up appointments and secure necessary authorizations. The role requires a healthcare-related bachelor’s degree and knowledge of discharge planning regulations, HIPAA, and EHR systems.

Qualifications

• Bachelor’s degree in healthcare-related field required.
• 2 years of healthcare experience or less than 1 year as a new graduate.
• Knowledge of discharge planning techniques and collaborative work style.
• Ability to manage multiple objectives with good prioritization.
• Computer and EHR literate; familiar with scheduling rules and HIPAA.

Responsibilities

• Coordinate post-acute services, home health, DME, transportation, infusion services, and community resources.
• Obtain payer authorization for level of care and length of stay; ensure timely documentation.
• Communicate with payers and families to support smooth transitions from hospital to home or another setting.
• Complete Medicare Notice of Hospital Discharge and regulatory documentation.
• Act as liaison among patients, families, providers, payers, and community resources.
