Job summary

Jefferson Health is seeking a Discharge Planner to coordinate safe and timely transitions of care for patients across multiple locations. The role requires collaboration with care managers, providers, and payers to arrange post-acute services and secure authorizations.

Responsibilities include documenting activities, assisting families, and ensuring regulatory compliance while guiding patients from hospital to home or another care setting.

Qualifications

• Bachelor’s degree in healthcare field with relevant clinical workflow knowledge.
• Experience coordinating post-acute services and payer authorizations.
• Knowledge of HIPAA and medical terminology.

Responsibilities

• Coordinate post-acute services including home health, DME, transportation and community resources.
• Obtain payer authorization for level of care and length of stay; communicate denials.
• Document discharge planning activities accurately in the medical record.
